Both health and dental insurance are available to all full- and part-time employees regularly scheduled to work 40 hours or more per pay period. Employees are eligible for health and dental insurance effective the first day of the month following their initial 30 days of continuous active employment. Only employees who choose to enroll have coverage.
The health insurance program offers two plan options: a PPO and an EPO. Both programs provide a comprehensive health benefit plan that covers hospitalization, physician visits, emergency care and outpatient diagnostic tests. Premiums are based on an employee’s work status, selected health plan and elected coverage.
The dental insurance program offers two plan options: a PPO and a DHMO. Both programs provide preventive, routine and major restorative work. Premiums are based on an employee’s work status, selected health plan and elected coverage.
