After being hospitalized for mental illness or substance abuse, the transition from a structured environment to home can be difficult. At Alexian Center for Mental Health, we offer you the help you need to overcome the challenges you might face after leaving the hospital. Our case management specialists will work with you to develop an individualized and comprehensive recovery plan that makes the transition back home as easy as possible. In-home and community-based supports can be provided on an ongoing basis to help monitor progress, link you with community resources and foster recovery success.
